YOUTH CORNHOLE REGISTRATION

In consideration of being permitted to participate in the Corn Hole Tournament (the “Tournament”) presented by the New Paris Applefest in collaboration with National Trail Soccer and in view of the risks inherent in such activity, the undersigned participant (the “Participant”) hereby agrees to release and waive and to indemnify and hold harmless, the New Paris Area Chamber of Commerce, National Trail Soccer, and their respective officers, trustees, coaches, employees, and agents from any claims of any nature whatsoever arising out of, or in any way relating to the activities thereof and Participant’s participation. The foregoing release, waiver and indemnification shall apply to, without limitation, participation in the Tournament and include but is not limited to claims by other Participants who may be injured or damages as a result of Participant’s conduct. The term “Participant” as used herein shall include the undersigned, any child whom the undersigned, as a parent or guardian, allows to participate in the Tournament, and any and all children, heirs, next of kin, assigns and personal representatives of any of them. Any electronic signature of this release shall have the same effect and shall be equally as binding as an original manual paper signature.
